Fees & Insurance

My fee per session is $150. I am sometimes able to offer a sliding fee scale based on availability.

I am contracted with the following insurance providers:

• Anthem

• First Choice Health Network | FCHN

• First Choice Employee Assistance Program (EAP)

• Kaiser PPO (not in network with Kaiser HMO or Kaiser CORE)

• LifeWise

• Modern Health Employee Assistance Program (EAP)

• Premera

If you have coverage with any of the above, I encourage you to find out what your coverage is for outpatient mental health services and to confirm that I am in-network with your plan. Email me if you need help with this, or have any questions about this process.

If you don’t have insurance with any of the providers above, but have out-of-network benefits, we can talk about how that might work. Sometimes, one possibility is that you would pay me directly and then submit invoices to your insurer using a receipt (Super Bill) that I provide to you.

Payment

I accept payment for session costs, or coinsurance/copays via credit card.
